Undo a Setting

I made the mistake of "okaying" a box that automatically fills in ur "name,

address, telephone number & e-mail address, in "office 2003", I'm guessing

that, that was where it was, but I don't know how to find & undo this 
setting, as it's a "pain in the u know where"!!!!  I okayed this while I
was 
re-loading my P.C., so I was pretty occupied with what I was doing, at
that 
time...
Can anybody please tell me, where & how to undo this mess !!! ????
