Data entered in fields keep disappearing (Dynamics CRM 3.0)

I am using Dynamics CRM 3.0 and I am noticing that entered data in certain 
fields keep disappearing for no apparent reason (notes on accounts,
contacts 
and op****tunities etc.). Has anybody else experienced this behaviour and
is 
there a known fix to this problem? It is really annoying.
