Clearing a form for each new record

I have a sales database that I'm building to keep track of sales functions,

contacts, etc.  I have one form that has multiple drop-downs based on data
I 
supplied and not a table/query etc.  It also has yes/no check boxes.  I
need 
everything to reset when the form is opened and when a new record is added
so 
that nobody accidentally uses information input by the previous user.   I 
don't know if that makes sense, but, if someone can figure it
out.....please 
let me know.
