Getting subform to work

I have a Main Form which loads the Application Table then I have a button 
that opens the Accidents Form.  The Accident Form is a Subform, I want the

Applicant ID from the Main Form to display and then be able to load the 
Accidents Table.  When I press the button I get a blank form.  I don't
think 
I have my settings right.
Any help would be appreciated.
