how to edit a prepared application form with access

My business uses a form for every client. I would like to scan it into my
computer and then be able to enter names addresses so forth, on screen and
print it out. it also has boxes that require calculations like inserting
a
date 15 days from today's date. I would like to be able to have those
calculations performed too. Can anyone point me to an online tutorial?
Or
know where i should go to look for information on how to set this up?
